Checklist before sending

• Print the name and account number of the creditor in question
• Check off or clearly print the specific reason for your dispute
• Clearly fill out all information, then sign the form where indicated
• Include one copy of a government-issued ID (driver's license, state or military ID)
• Include one copy of a utility bill, bank, or insurance statement
• Do not include original copies of your supporting documents
• Mail the form to the appropriate bureau, or file online
• There is no charge for submitting a dispute
• Make a copy for your records
