Negotiate Hospital Bills

How to negotiate hospital bills

Most people don't know that hospital chargemaster prices are negotiable. Hospitals expect insurance to negotiate down, so the sticker price is inflated 3-10x the actual cost. When you pay cash, you can negotiate the same discounts — or better.

The 4-step negotiation process

  1. Request an itemized bill. You have a right to this. Review every charge — up to 80% of hospital bills contain errors.
  2. Ask for the cash-pay rate. Hospitals have a "chargemaster" price and a "cash-pay" price. The cash price is typically 40-60% lower.
  3. Apply for financial assistance (charity care). Nonprofit hospitals are required by law to offer financial assistance. Income thresholds vary but many cover 200-400% of federal poverty level.
  4. Set up an interest-free payment plan. If you can't pay in full, hospitals will accept monthly payments — often with no interest and no collection threat as long as you're paying.

Negotiation script

"Hi, I'm calling about my bill for [AMOUNT] from [DATE OF SERVICE]. I'd like to discuss payment options. I'm unable to pay the full amount, but I'd like to work out a reduced rate. I understand there's a cash-pay discount — can you tell me what that would be for my bill? I'd also like to apply for your financial assistance program. Can you send me the application?" [IF THEY OFFER A DISCOUNT] "Thank you. I can pay [OFFERED AMOUNT] in full today if we can settle this and close the account." [IF THEY CAN'T REDUCE] "I understand. Can we set up a payment plan of [$X/MONTH] so I can pay this off without it going to collections?"
